How To Get Help
Pathway into Home and Community Care
If you are not currently receiving any Home and Community Care (HACC) support, call the Commonwealth Respite and Carelink Centre on 1800 052 222 and they will discuss your concerns and refer you on to be assessed. The Regional Assessment Services assessor will then visit you in your home, at your convenience.
This information is then given to a service provider (the agency who will assist) who will
contact you and place the support necessary to allow you to remain living independently
in your home.
If you are an existing Home and Community Care (HACC) client wanting to receive additional
services, please contact your service provider (e.g. Southern Districts Support Association)
and they will request a reassessment of your current services. The Regional Assessment
Services assessor will visit you in your home, at your convenience.
This information is then given to your service provider to allow them to provide you with
ongoing support, or in some cases the support required changes and the
service provider
can amend the support you are currently receiving to allow you to remain living independently
in your home.
